Why CBT and EMDR Can Leave High-Functioning Anxiety Unresolved
If you have ever googled the best therapy for anxiety, you were likely flooded with articles praising Cognitive Behavioral Therapy (CBT) or traditional EMDR. They are treated as the ultimate gold standards in the traditional medical model. They promise that if you can just identify your irrational thoughts, reframe them, and fill out a few thought logs, your internal chaos will fade.
So, you try it. You read the books. You fill out the worksheets.
But if you are a highly logical, deeply capable woman who grew up navigating emotionally immature or manipulative family dynamics, you likely found that these treatments didn't fully resolve things. Traditional top-down protocols push you deeper into your analytical brain while leaving your underlying relational wounds completely untouched.

Traditional CBT operates on the assumption that your anxiety is simply a glitch in your logic. It asks you to look at your fear of family conflict or your automatic urge to keep the peace, and label it as a "cognitive distortion."
But for the adult daughter of a narcissistic or emotionally immature parent, reading the room and anticipating everyone else's needs wasn't a distortion. It was a highly intelligent, completely necessary survival strategy.
When a therapist hands you a thought log and tells you to rationalize away the tight knot in your stomach, your system experiences it as a profound disconnect. Your intellect already knows you are safe. Your logical mind knows you don't need to fix your partner’s bad mood or spend three hours over-preparing a single work email.
When you try to logic away a physical survival state, you create a painful internal split. You end up sitting with your worksheets feeling entirely broken, wondering why you can't just think your way out of years of learned safety adaptations.
The truth for many of my clients is that the danger isn’t imaginary. CBT asks you to challenge if your assumptions are 100% true. For example, a standard worksheet might ask: "Is it 100% true that if you say no, your mother will never forgive you?"
But that question completely misses the point. The bigger question is: What has been true in your life that created the absolute need to keep the peace at the cost of your own soul?
That inner wound holds the truth of your unique experience—past, present, and future. For most of my clients, it is true that saying no will bring a real-world consequence. There will be an immediate guilt trip, emotional withdrawal, triangulation, or passive-aggressive sulking. Therefore, in my practice, we don't pretend your boundaries won't cause friction. We don't treat your fears as a false assumption. Instead, we start from a posture of curiosity, acknowledgment, and respect for the survival patterns your body built to keep you safe.
The Hidden Overload of Standard EMDR
Similarly, standard EMDR protocols can often be a severe mismatch for masked neurodivergent women navigating adult ADHD, AuDHD, or highly sensitive nervous systems. Traditional EMDR’s rigid protocols require rapid, continuous bilateral stimulation, forcing your brain to engage in fast eye movements or hold rapid tappers.
For a sensitive nervous system that already struggles with sensory filtering, this rapid tracking can cause intense sensory flooding and executive collapse. It asks your brain and body to multi-task during deep trauma processing when your system simply isn’t wired to do so without intentional adaptations.
In fact, I experienced this mismatch myself as a client. Long before I was diagnosed or screened for my own neurodivergent traits, I sat through traditional EMDR protocols that left my system completely overwhelmed. My provider was a seasoned, highly capable clinician and an approved EMDR supervisor—but the rigid tools simply weren't built with my specific brain in mind.
Evaluating for underlying neurodivergence masking is a fundamental part of my clinical rule-out process. I never assume a system is neurotypical. Rather than treating a client's traits as a fluke discovery halfway through treatment, we honor your unique neural wiring from day one.
